Victoria Hospital Foundation recently partnered with the Winnipeg Regional Health Authority on a groundbreaking initiative to enhance care for home care clients and create greater efficiencies for staff.

Victoria Hospital has seen a 24.8% improvement in discharge times, with many patients leaving a half or full day earlier than before.

Faster, Clearer Communication
“Timely communication between the office and staff has always been our problem,” said Luba Bereza, Director of Centralized Home Care Operations. “We were playing phone tag… now, we just update it on a computer and it’s instantly on their phone.”
The project also saved over $33,000 in paper costs in its first year alone.
The Winnipeg Regional Health Authority manages the largest home care workforce in Manitoba, with 2,200 staff conducting approximately 16,000 visits per day.
Simple updates—like reassigning a client’s primary contact—now happen seamlessly, and staff can join region-wide safety huddles and receive real-time updates.
“What this app allows us to do is have one source of truth for communication,” said Bereza.
Staff Feedback and Improved Client Care
Staff feedback has been overwhelmingly positive.
Matthew Jacobi, a certified healthcare aide, shared:
“It has been quite user friendly and helpful all around… The security risk of leaving a piece of paper laying around isn’t there anymore.”
Jacobi also noted improvements in client care:
“Now you are going in and checking off each task as you go. It takes just a second to make sure we are getting all tasks completed.”
Setting a New Provincial Standard
This project is now being evaluated as the new standard for home care across Manitoba.
Strengthening the Entire Healthcare System
This initiative has increased home care capacity, reduced fall risks, and eased transitions from hospital to home. These investments reduce hospital visits and improve healthcare across the system.
